We offer a Heart Disease Focused Wellness program for our members who have heart disease, which includes high blood pressure and heart failure.
What is high blood pressure?
High blood pressure happens when the force of blood being pumped through your blood vessels is too strong. High blood pressure affects approximately 26.6% of adults in Minnesota. That's more than 1.3 million people.
What is heart failure?
Heart failure, also known as congestive heart failure, happens when your heart can’t pump enough blood for your body’s needs. Your heart may not be able to fill up with enough blood or it may be too weak to pump the blood properly.
What does this program do?
This program can help you and your primary care provider manage your heart disease. This program was designed using a model called COACH, which is an innovative way to help people take charge of their health. When you join the program, you will choose a goal to work on to help you meet your needs. As part of our program, a Focused Wellness nurse is available to answer your questions. They can help you do the following:
- Improve your quality of life and health status
- Reduce your risk of experiencing adverse cardiovascular events
- Build skills to change and sustain the behaviors that influence controllable risk factors
If you say it’s okay, we may talk to your health care provider about your condition. Your health care provider can help you manage your heart disease. Please talk to them any time you think your condition is not under control.
High Blood Pressure Action Plan – Develop an action plan to help with day-to-day management and to know what to do in an emergency.
