We offer an Asthma Focused Wellness program for our members who have asthma.
What is asthma?
Asthma is chronic swelling of the airways. It may seem to go away, but the swelling can flare up at any time.
What does this program do?
This program can help you learn to manage your asthma and understand why managing it is important. It was designed using a model called COACH, which is an innovative way to help people take charge of their health. When you join the Asthma Focused Wellness program, you will choose a goal to work on to help you meet your needs. As part of the program, a nurse is available to answer your questions. They can help you do the following:
- Understand your asthma
- Understand your asthma medicines
- Get the supplies and equipment you need
- Find and receive care from special providers who work with asthma
If you say it’s okay, we may talk to your health care provider about your condition. Your health care provider can help you with your asthma. Talk to them any time you think your asthma isn’t under control.
Asthma Action Plan – Develop an action plan to help with day-to-day management and to know what to do in an emergency.
Asthma Camp for Kids – PrimeWest Health encourages all eligible members to attend a qualified summer health camp. These camps are a great chance for youth to have fun, learn how to take control of their conditions, and form a community of friends with similar concerns.
