Care coordination and case management are processes that link members to services and resources to promote optimal health. These can include disease management, home and community based supports, Women, Infants, and Children (WIC) services, behavioral health services, assistance with health care directives, and much more.
The mission of PrimeWest Health Care Management is managing care for PrimeWest Health members across the continuum to improve health, enhance care experience, and reduce health care fragmentation, while empowering them to understand and access quality, cost-effective health care.
