Password Reset

  1. Go to the provider web portal home page.
     
  2. Log in to the provider web portal by selecting Login from the brown bar at the top of the page. Then, select Provider Web Portal.
     
  3. Click Forgot password under the “Remember my username” checkbox.
     
  4. Enter your username on the “Forgotten password?” screen. Click Submit.
     
  5. You will receive an email from PrimeWest Health (no-reply@primewest.org) with the subject line “PrimeWest Health: You have requested a password reset.” Click on the link in the email or copy the URL into your browser to reset your password.
    1. You must reset your password within 60 minutes of receiving this email. If you wait longer than 60 minutes, you will need to start back at step 3.
       
  6. Enter your new password in the “New password” field. Enter it again in the “Confirm password” field. Click Submit. Keep the following in mind when resetting your password:
    1. You cannot use any variation of your last password
    2. Passwords must be at least eight characters long and contain at least one uppercase letter, one lowercase letter, one number, and one special character
       
  7. Your password has been changed. Return to the log-in page to log in with your username and new password.
