Password Reset
- Go to the provider web portal home page.
- Log in to the provider web portal by selecting Login from the brown bar at the top of the page. Then, select Provider Web Portal.
- Click Forgot password under the “Remember my username” checkbox.
- Enter your username on the “Forgotten password?” screen. Click Submit.
- You will receive an email from PrimeWest Health (no-reply@primewest.org) with the subject line “PrimeWest Health: You have requested a password reset.” Click on the link in the email or copy the URL into your browser to reset your password.
- You must reset your password within 60 minutes of receiving this email. If you wait longer than 60 minutes, you will need to start back at step 3.

- Enter your new password in the “New password” field. Enter it again in the “Confirm password” field. Click Submit. Keep the following in mind when resetting your password:
- You cannot use any variation of your last password
- Passwords must be at least eight characters long and contain at least one uppercase letter, one lowercase letter, one number, and one special character
- Your password has been changed. Return to the log-in page to log in with your username and new password.
