Eligible Providers

A hospice organization may enroll as a PrimeWest Health hospice provider if it is licensed and certified for Medicare as a hospice by the Minnesota Department of Health (MDH). In order for hospice services to be covered, a plan of care must be established.

A hospice may use contracted staff to supplement hospice employees during periods of peak patient loads or other extraordinary circumstances. The hospice remains responsible for the quality of services provided by contracted staff.
