Contracting
PrimeWest Health's participating providers are facilities that have entered into a contract with PrimeWest Health and are part of our network.
The PrimeWest Health network consists of many provider types and specialties. If your facility is not currently in the PrimeWest Health network, please read the non-participating provider information below.
Non-Participating Providers
Non-participating providers are required to obtain a Service Authorization for most services. Please review the Service Authorization/Notification Forms page for more information section. If you would like to be considered for contracting with PrimeWest Health, please complete the Participation Request – Contracted Facility form.
Credentialing is not required for non-participating providers only. Credentialing is required for participating providers.
New, non-participating providers (i.e., providers that have not contracted to be part of PrimeWest Health’s network) must register before submitting claims. For more information about submitting a claim or payment request for health care services or supplies, review the information on our Out-of-Network Facility Registration Requirements page.
Participating Providers
If you are a participating provider, please complete the contracting forms for participating providers as applicable or as requested by PrimeWest Health.
