Member Identification (ID) Cards
When members are enrolled in PrimeWest Health, they receive a PrimeWest Health identification (ID) card with their Person Master Index (PMI) number printed on it. PrimeWest Health uses the State PMI number as the member number.
- All member PrimeWest Health ID cards are issued in the same format and contain the same general information.
- PrimeWest Health ID numbers do not change and follow members through any changes in program, eligibility, or address.
- Each member of a household is issued his/her own individual PrimeWest Health ID card with the program s/he is enrolled in printed on the card.
- The PrimeWest Health ID card does not include information about spenddowns, other health insurance coverage, Medicare coverage, or member restrictions.
- Verify eligibility each month for each member or on the day service is rendered using the following:
- PrimeWest Health’s web service (CORE certified) – Note: Your clearinghouse and system software program must support the eligibility transaction in the standard electronic data interchange (EDI) transaction ASC X12 270/271.
- Information provided in an EDI 271 transaction is not a guarantee of payment or coverage in any specific amount. Actual benefits depend on various factors, including compliance with applicable administrative protocols, date(s) of services rendered, and benefit plan terms and conditions.
- PrimeWest Health web portal. Prior registration is required using the Web Portal Registration Form.
