Caregiver Support Program
What is the Caregiver Support Program?
PrimeWest Health’s Caregiver Support Program helps people who provide care to members with Alzheimer's disease or related dementias. This program provides caregivers with the support, coaching, education, and resources they need to confidently provide care to members.
Who can enroll?
PrimeWest Health members and their caregivers can enroll in the Caregiver Support Program.
What does this program provide?
- Support from nurse care coordinators or a community health worker
- Educational materials that can help you and your caregiver understand your condition
- Assistance with finding resources or services
- A free caregiver binder to help organize important information
To enroll in the Caregiver Support Program or learn more, please call Member Services at 1-866-431-0801. TTY users call 1-800-627-3529 or 711. These calls are free. If your caregiver is not already listed as an authorized representative, please fill out and return the Designation of an Authorized Representative Form.
