Performance Improvement Projects (PIPs)
Performance Improvement Projects (PIPs) are projects designed to improve members' health or the way they access care. These projects aim to improve any of the following:
- Clinical components – specific things members can do to improve their health
- Organizational components – aspects of PrimeWest Health that affect accessibility, availability, comprehensiveness, and continuity of health care
- Member components – members' perceptions about the quality of PrimeWest Health's services
Current PIPs
Postpartum Depression Screening
The goal of this PIP is to increase the number of new mothers screened for postpartum depression. It aims to do this by performing member outreach, provider outreach, and working with Public Health agencies. This PIP has a focus on racial/ethnic disparities and has a goal of closing the gap across several populations. All Families and Children and MinnesotaCare members will be included in our efforts.
Depression and Diabetes
The goal of this PIP is to increase the rate of members with diabetes who are seen for an eye exam by three percentage points in each population. A special focus of this PIP will be on members who are diagnosed with both diabetes and depression. To meet the goal, we will perform member and provider outreach, provide education, and work with county case managers. All PrimeWest Senior Health Complete and Prime Health Complete members will be included in our efforts.
