Child and Adolescent Well-Care Visits (WCV)
Child and adolescent well-care visits are similar to Child and Teen Checkups (C&TCs) and the Federal Early and Periodic Screening Diagnosis and Treatment (EPSDT) program, except they don't require as many elements. For a visit to be counted as a well-care visit for this Healthcare Effectiveness Data and Information Set (HEDIS®)1 measure, documentation in the medical record must include notation of a visit to a primary care or obstetrics/gynecology (OB/GYN) practitioner, the date the visit took place, and evidence of all of the following:
- A health history
- A physical development history
- A mental development history
- A physical exam
- Health education/anticipatory guidance
Who was measured?
PrimeWest Health members ages 3 – 21 as of December 31 of the measurement year who were continuously enrolled during the measure year.
What can providers do?
Adolescent well-child visits is an area that can use improvement. PrimeWest Health urges providers to take advantage of every opportunity when children or adolescents are in your clinic. If they come in for a sick/problem-based visit, turn it into a full well-care visit that includes a health history, mental and physical developmental histories, physical exam, and anticipatory guidance. If that isn’t possible, encourage members or their parents/guardians to make follow-up well-care visits.
